It is a short and easy process – all you have to do is fill out the fields on a PDF application form that you can email or fax in. A customer service representative will be in touch with you as soon as we have your information. Please be sure to include an accurate phone number so that we can contact you immediately with any questions.

HOW TO COMPLETE THE FORM:

  1. Open the PDF form above.
  2. Complete the fields.
  3. Save As - rename and save the file on your computer to keep for your records.
  4. Print the application and sign it.
  5. Send the form to us via:

It usually takes a couple of business days to verify your application and setup your account. You will receive your account information via email as soon as your account is ready to go.
